Near Miss

Objective: To report any kind of Near Miss occur in Project/Site during day-to-day activity. It allows user to create, review and approve/reject Near Miss in the system.

Below steps to be followed to complete an incident report:

  1. Report Near Miss : To create/report new Near Miss. Any employee working on the Project/Site, who has access to the system can report an Near Miss.
  2. Review Near Miss & Identify investigation Team member : Initial Near Miss Report review task is sent to Cluster HSE Head. Cluster HSE Head review Initial Near Miss Report, Identify investigation Team member and submit or can directly add Action Item(s)/CAPA and Close it. Closed Initial Investigation Report’s CAPA will be assigned to selected responsible person.
  3. Near Miss Investigation : Investigation will be conducted by any user who is selected as investigation Team member. In Investigation details, Action Item(s)/CAPA must be added which will be assigned to selected responsible person after Investigation final reviewed and closed.
  4. Review Near Miss Investigation by Cluster HSE Head : Cluster HSE Head will review investigation report and if report is satisfactory then report can be closed otherwise the report will be sent to investigation team to revise. Closed Investigation will be assigned to selected responsible person.

REPORT NEAR MISS

  1. Go to Incident/Near Miss | Report Near Miss screen (Image 2A).
  2. Enter the Business Unit, Region, Sub-Region, Project, Site, Department, Contractor in auto complete text box. (Business Unit, Region, Sub-Region, Project, Site, Department, Contractor will be displayed automatically when have single value or first 3 characters are typed)
  3. Click “Submit” button to save the data and send the Initial Incident Report for review to “Cluster HSE Head”.

Note :

  • Near Miss can be reported by any employee working in plant.
  • The fields marked with * are mandatory to be filled.
