Incident/Accident

Objective: To report occurrence of any incident within the Project/Site during day-to-day activity. It allows user to report, review and approve/reject incident using the system.

Below steps to be followed to complete an incident report:

  1. Report Incident : To create/report new incident. Any employee working on the Plant, who has access to the system can report an incident.
  2. Review Incident & Identify investigation Team member : Initial Incident Report review task is sent to Cluster HSE Head. Cluster HSE Head review Initial Incident Report, Identify investigation Team member and submit or can directly add Action Item(s)/CAPA and Close it. Closed Initial Investigation Report’s CAPA will be assigned to selected responsible person.
  3. Incident Investigation : Investigation will be conducted by any user who is selected as investigation Team member. In Investigation details, Action Item(s)/CAPA must be added which will be assigned to selected responsible person after Investigation final reviewed and closed.
  4. Review Incident Investigation by Cluster HSE Head : Cluster HSE Head will review investigation report and if report is satisfactory then report can be closed otherwise the report will be sent to investigation team to revise. Closed Investigation will be assigned to selected responsible person.

REPORT INCIDENT

  1. Go to Incident/Near Miss | Report Incident screen (Image 1A).
  2. Enter the Business Unit, Region, Sub-Region, Project, Site, Department, Contractor in auto complete text box. (Business Unit, Region, Sub-Region, Project, Site, Department, Contractor will be displayed automatically when have single value or first 3 characters are typed)
  3. Click “Submit” button to save the data and send the Initial Incident Report for review to “Cluster HSE Head”.

Note :

  • Incident can be reported by any employee working in Project/Site.
  • The fields marked with * are mandatory to be filled.
